Immunizations

Current Deadlines:

In order for your child to avoid being excluded from school, parents/caregivers must have all immunizations up-to-date with medical documentation, or complete a Statement of Exemption signed and returned to the school nurse by:

August 29, 2022

In accordance with the immunization requirements from the Pennsylvania Department of Health and State Board of Education all students grades K-12 must have the immunizations listed below to attend school.  A record of immunizations may be dropped off at your student's building main office.  Contact your health provider or call 1-877-PA HEALTH for more information.

Required immunizations:

DTP/DTAP - 4 doses (1 dose on or after their 4th birthday) 
POLIO - 4 doses (4th dose on or after 4th birthday & at least 6 months after last dose)
MMR - 2 doses (measles, mumps, & rubella)
HEPATITIS B - 3 doses
 VARICELLA - 2 doses (or evidence of chicken pox disease
 TDAP - 1 dose (required at 11-12 years of age to enter grades 7-12)
 MENINGOCOCCAL/MCV  - 2 doses (1st dose at 11-15 years of age, required to enter grade 7, 2nd dose required at age 16 or entry into 12th grade.)

Immunization Records & Physical

Physical must be completed after 8/31/22 and are required in grades K, 6, & 11. You may submit a copy of the physical & immunization records provided by the student's physician or use the state form to be completed by medical provider.

 

Dental Exams

PA Public School Code requires dental exams in grades K, 3, & 7. Exams can be accepted after 8/31/22.
