Important Information

All Sports Physicals will be held on Friday, June 7th!!  Click on the Full Physical Packet to the right for more information!

In order to play a fall sport, all physical paperwork must be turned into the Athletic Trainer by August 1st!!

School Physical Fee - $25.00 

• If you attend the School Physical, the fee is due with this physical packet
• If you go to your own doctor, you are exempt from this fee
• Payable by CHECK ONLY to TVSD

High School Activity Fee - $75.00 per sport

• Fall Sports activity fee is due with this physical packet
• Winter and spring sports activity fee will be due with the recertification packet
• Lacrosse, Swimming and HS Wrestling are exempt from this fee since they are self-funded
• Payable by CHECK ONLY to TVSD

Middle School Activity Fee - $50.00 per sport

• Fall sports activity fee is due with this physical packet
• Winter and spring sports activity fee will be due with the recertification packet
• Payable by CHECK ONLY to TVSD

Recertification packets will NOT be accepted for the fall.  This must be a new physical dated after June 1, 2013.

Cups will no longer be provided by the Athletic Department.  Please make sure you bring your own water bottle to all practices and games.  If you are interested in purchasing a TV water bottle, please stop in the Athletic Training Room.​​
